Old gasket

Reusing an unsuitable or deformed gasket can lead to a loss of seal integrity.

Poor alignment

If piping is forced into position using bolts, mechanical stresses are created.

Forgotten connection

Small instrument lines, pressure taps, or drain/vent points may remain loose after the work is completed.

Lack of final inspection

Completing the mechanical work does not mean the installation is ready. A final leak test must follow.

Use a checklist with all the points that were loosened or affected so that none are omitted before restarting.
